Professional Liability Insurance
Options: Paid 100% by employer 

Eligibility: All Therapists and Therapist Assistants

Short Term Disability, Cancer, Accident and Critical Illness

Options:  Colonial Life (100% paid by the employee).  Colonial Life offers supplemental policies that employees can design based on their needs.  Coverage is portable and payroll deduction is offered.  

Eligibility:  Full time and part time employees.  Eligibility to enroll  begins the first of the month following date of hire.  Policy effective dates are subject to when coverage is bound.

Pre-paid Legal and ID Theft Insurance

Options:  Legal Shield (100% paid by employee).  This coverage offers legal services in addition to preparation of a basic will at no cost to the employee.  Payroll deduction is offered.  

Eligibility:  Full time and part time employees.  Eligibility to enroll  begins the first of the month following date of hire.  Policy effective dates are subject to when coverage is bound.

401k

Options:  American Funds.  Graduated vesting.  Participants must be at least 21 years of age.  

Eligibility:  Full time and part time employees.  Once employed for 60 days, employee may enroll based on quarterly enrollment schedule- Jan 1, Apr 1, July 1 or Oct 1.
